Starlight Healthcare welcomes referrals from individuals, families, social workers, local authorities, NHS and healthcare professionals, commissioners, advocates and other partner organisations.

We provide person-centred services across Southampton, Stafford and Bristol. Every referral is considered carefully, with the person’s safety, needs, preferences, strengths and desired outcomes at the centre of our decision-making.

Our Referral Pathways

The Adult Supported Accommodation

Our adult supported accommodation services are designed for people who need personalised support to live as independently as possible.

We can consider referrals for adults with needs including:

  • Learning disabilities
  • Autism
  • Mental health needs
  • Acquired brain injuries
  • Physical disabilities
  • Complex health or support needs
  • Behaviours that may challenge
  • Support needs relating to daily living and community participation

Support is developed around the individual’s abilities, preferences, communication needs, risks and personal goals. We work collaboratively with the person, their family or representative, social workers, healthcare professionals and housing partners.

Domiciliary and Community-Based Care

Our domiciliary care service supports people to remain safe, well and independent in their own homes.

Support may include:

  • Personal care
  • Medication support
  • Meal preparation
  • Domestic support
  • Companionship and community access
  • Support with appointments and daily routines
  • Respite and short-term support
  • Live-in care
  • Support for physical disabilities and neurological conditions
  • Bespoke support for complex health and social care needs

Care can be arranged according to the person’s assessed needs, from scheduled visits to more intensive packages of support.

Supported Accommodation for Young People Aged 16+

Our young people’s supported accommodation service provides a safe and nurturing environment for young people preparing for adulthood and greater independence.

We work with placing authorities and professionals to consider referrals based on the young person’s age, assessed needs, risks, placement plan and desired outcomes.

Support may focus on:

  • Maintaining safety and emotional wellbeing
  • Building trusting and consistent relationships
  • Developing practical independent-living skills
  • Education, training and employment
  • Budgeting and managing money
  • Maintaining a home and preparing meals
  • Accessing health and community services
  • Building positive relationships and support networks
  • Preparing for adulthood and future accommodation

Our supported accommodation for 16- and 17-year-olds operates within the Ofsted regulatory framework and the Supported Accommodation (England) Regulations 2023. Adult regulated care services are considered separately within the appropriate CQC framework.

What Happens After a Referral?

Submitting a referral does not automatically guarantee that we can offer a service. Every decision is
based on assessment, compatibility, safety, funding, regulatory requirements and availability.

Referral Received

Our team reviews the information provided and may contact the referrer to clarify the person’s needs, risks, funding arrangements and preferred location.

Initial Suitability Review

We consider whether Starlight Healthcare can safely and appropriately meet the person’s needs. Where accommodation is required, we also consider location, compatibility and current availability.

Assessment

We speak with the person and, where appropriate, their family, representative and involved professionals. The assessment considers the person’s needs, strengths, preferences, communication, safeguarding risks and desired outcomes.

Proposed Support

If we can meet the person’s needs, we develop a proposed support approach. This may include staffing arrangements, accommodation options, risk-management measures, expected outcomes and indicative costs.

Transition and Start of Service

Once the placement or care package is agreed, we create a planned transition. We work with everyone involved to make the move or start of support as safe, comfortable and well-coordinated as possible.
